37 / The car-crash victim's family-relationship puzzle

A horrific car crash. The driver dead, his son injured terribly. The dead man's son is taken to hospital and needs to be operated on. 
The surgeon exclaims, "Oh my god - my son, my poor son!" 


What's going on here?


If you had problems with this puzzle you make a common mistake and you are excused for thinking that surgeons - just like many other persons of authority - generally are male; probably most of us are conditioned to think this way. Thus most people only consider scenarios where the boy was driving in the car with an adoptive father, he had two gay fathers etc. But that is not the case, he only had one father who then died in the crash - however, the surgeon is female and the boy's mother. (Myself, I don't know any female surgeons - but then, 

I don't know any surgeons at all.)
